Susceptibility Profile of Nitrofurantoin and Fosfomycin among Carbapenem-resistant Enterobacteriaceae Isolates in UTI from a Tertiary Care Hospital

UTI is one of the most common infections requiring antibiotic treatment and hospitalization. The rising trend in multidrug resistance to commonly used antibiotics has reduced therapeutic options for treating these infections. Reexploring older like nitrofurantoin fosfomycin provide help combat resistance. This prospective study was conducted Department Microbiology, SRM Medical College Hospital Research Center, from July 2021 February 2022. included only clean catch midstream urine isolates Escherichia coli Klebsiella pneumonia hospitalized patients outpatients. Standard microbiological procedures were process samples. Direct gram stain conventional biochemical reactions performed identify isolates. antimicrobial susceptibility testing carried out by Kirby Bauer disk diffusion method Minimum Inhibitory Concentration E- test gradient fosfomycin. MIC determined Micro Broth Well Dilution according CLSI guidelines 2021. Among 150 samples, 107 (71.3%) higher than 43 (29%). Carbapenemase production seen 58 (63.04%) disc method. positive carbapenemase producers, E. found be 33 (56.8%), 25 (43.1%). Fosfomycin rates E reported high coli, ranging 0.5-1mg/L. less susceptible 16-32mg/L. Only 7(21%) showed 1-4µg/ml broth microdilution. 21 (63.63%) 11(44%) have an intermediate category with 8-32µg/mL. A 64- > 256µg/ml shown 5 (15.15%) 14 (56%) pneumonia. Older medications may resurface as useful choices current grow.
